1. 먼저 아래 첨부된

(1) 보호자 동의서(parental consent form)

(2) 개인정보이용동의서(the use of personal information)

파일을 다운로드 받고, 작성 후 서명까지 완료해 주세요.

2. 증명사진, 여권 사본, 신분증 사본을 준비 후, 설문지를 작성 해 주세요.

*영어, 일본어, 한국어, 중국어로 작성 가능합니다.
